Participants have access to this ongoing educational tool for one year from the date of training. Access can be renewed annually for a modest fee.
Participation in the highly interactive simulation-based face-to-face CTI® training experience. Attendees have the opportunity to learn how to incorporate CTI® tools into the role of Transitions Coach® so that they become second nature. Participants also have access to our expert training team to get individual questions answered.
Invitation to our monthly CTI® Community Learning calls. During the peer-to-peer learning calls colleagues bring challenging cases, new techniques and best practices, and provide positive reinforcement for the Transitions Coach® role. These calls are also an opportunity to learn about upcoming changes in health policy to help organizations be prepared. Joining these calls is a great way to stay connected with colleagues and the Care Transitions Program® team.
PLEASE NOTE:
Completing the Care Transitions Intervention® training does not authorize you to train others. All training must be arranged through and taught by The Care Transitions Program® training team.
Our Training Menu

Core CTI® Training
Core CTI® Training provides new Transitions Coaches® with the fundamental skills of the role and the CTI® model. Core CTI® Training is offered throughout the calendar year. These trainings last 1 day and occur in Aurora, Colorado (between the city of Denver and Denver’s airport). The Care Transitions Program® reserves the right to modify training dates depending on participant demand and/or availability of member of our training team. Please do not assume you are enrolled in training or make your travel reservations until you have received confirmation from Sue Rosenbek. Trainees must cover their own travel expenses.
We also can bring the CTI® Core Training to you (referred to as an “on-site” training experience), These trainings last one-and-a-half days and are hosted by a single organization requesting a private training event for Transitions Coach® trainees. These trainings are for groups of up to 35 trainees. The host site provides a suitable training venue that allows for small group learning and refreshments.

Advanced CTI® Training
Advanced CTI® Training is designed for Transitions Coaches® that have previously been trained in the Care Transition Intervention® and have at least three months practice in the field following the CTI® Core Training. This is a one-and-a-half day course with two areas of focus: First, we will train on the new evidence-based Family Caregiver Enhanced CTI® training experience aimed at how to better engage family caregivers. Second, we have heard from our CTI® Learning Community that there is demand for new opportunities to refresh and expand the core skills of coaching, and have built a new interactive curriculum accordingly.
The Advanced CTI® Training can also be brought to your organization (available on-site). The on-site CTI® Advanced Trainings, last one-and-a-half days and are hosted by a single organization requesting a private training event for Transitions Coach® trainees. These trainings are for groups of up to 35 trainees who have undergone the Core CTI® Training. The host site provides a suitable training venue that allows for small group learning and refreshments.

Please note: The Care Transitions Program® has the EXCLUSIVE authority to provide training on the Care Transitions Intervention®. Please do not accept training offers from other entities.
