Share Your Story

We can learn a great deal from listening to each other’s stories. Individually, these stories can be easily ignored. However, collectively, our stories and anecdotes can be quite powerful. The Care Transitions Program® would like to invite you to share your story, and we would then like to harness the power of these stories to effect change in our health care delivery.

If you are a patient, caregiver, or healthcare provider with an experience involving a care transition (meaning the hand-off that occurs across health care settings), please share your story with us. Please use the form below to tell us about your experience, and click the “submit” button when you are finished. You may identify yourself or remain anonymous. We may post your story on our website. We will also share the stories collectively in national forums with health care leaders, accreditation agencies, and health care policymakers.

Please note that we are not able to resolve disputes or provide legal advice. You may click here to find additional resources that will inform you of your rights and how to file a grievance.

Thank you for being part of our community of change!
